When you sign up for Patient Portal, a representative from our office will print a card with the Patient Portal URL and your Username and Password. This will be sent to you via email.

Type the URL (https://midm.phiportal.com)within your browser’s address bar and add your unique Username (usually your email address) and Password into those fields.
Click the Log In button to log in to your Patient Portal account.

We request that you:
Contact your current or previous dental office and request that they send X-rays to our office by email. We require a current full mouth series or panoramic X-rays for our office to provide proper diagnosis and treatment.
Take a photo of the front and back of your insurance card and email it to us so that we may research your information and obtain a general breakdown of benefits as a courtesy for you. You MUST also present your state issued ID.

If you will be using dental insurance, we will submit your claims for you electronically. Payment for services will be expected at the time of service.

**New patient(s) are required to fill out and return new patient registration information thru our secure patient portal 48 hours prior to your appointment.**

Note: Your visit may include a Cone Beam Computed Tomography also known as CBCT or 3D image of the head, neck and oral cavity. The fee is $440 and it encompasses taking the image, interpretation by a Radiologist and Doctor Harris’ treatment planning and reporting findings.

We accept cash, check, visa, master-card, discover, american express, HSA cards, flexible spending accounts and CareCredit.
